Enrollment Process

If your son or daughter is interested in joining cadets please follow the step by step instructions outlined on this page.  If you have any questions regarding the enrolment process or the cadet program please contact the recruit officer at recruito@cycloneaircadets.com or call and leave a message at 613-845-0166.

Step 1.

Before starting to fill out the paperwork listed below it is important to compile a few things to make filling the forms in easier.  These items are listed below:

  • Provincial/Territorial health card
  • Any one of the following proofs of identity:

o   Valid Canadian passport

o   A birth certificate issued by Canadian provincial, territorial, or federal government agency

o   A valid identification card, visa or permit (work, study, resident, etc.), with picture, delivered by a Canadian federal government agency which legally authorizes the holder to reside in Canada for an extended period of time.

o   A valid identification card, with or without picture, issued by a Canadian federal, provincial, or territorial government agency

*Note
a baptismal certificate issued by a religious order will not be accepted as a proof of identity

  • When applicable, a copy of the relevant section of the court decree or judgement or separation agreement defining the legal guardian access rights; and
  • When applicable, a copy of the relevant section of the court decree or judgement imposing restrictions, obligations and/or conditions on the cadet.

 

Step 2.

Once all the material is compiled a form is required to be filled in as part of the enrolment process.  This form can be found linked below this information.  The preferred method is to fill it in via computer and then print it and bring it in during one of our training nights.  If you do not have access to a computer hooked up to a printer then hard copies of this form can be obtained from the recruit officer during a regular training night.

The form required for enrolment is the DND 1158 – Application for Enrolment.

There is the possibility that an additional medical screening form may be required if you answered "yes" to any of the medical questions on the DND 1158 - Application for Enrolment form. This form is known as the DND 2570 "Detailed Health Questionnaire" and can be found at the following link:

Step 3.

Once all of the correct forms are filled out and the proper documentation from step 1 is compiled the entire package can then be taken to one of our regular training nights.  The information regarding time and location can be found under the “information” tab on the left hand side of the website.

It is important that parents accompany their children in with the required forms on the first night as there is the potential that there may be more paperwork required depending on the completion of the paperwork as well as the responses to certain questions on the application for enrollment.

***Once the forms have all been submitted they will be reviewed by our administration officer who will then pass them on to our commanding officer who will be the authority on approval of enrolment.  Unfortunately potential cadets will not be able to participate until all forms, proof of health insurance and proof of citizenship are submitted.
